About Us

We Focus Exclusively on Support Coordination

Support Connection is a New Jersey state-approved Support Coordination Agency based in Red Bank, Monmouth County. We are a "released" agency, meaning we approve our own service plans. At Support Connection we are dedicated to providing support coordination services to assist individuals with developmental disabilities and their families in gaining access to needed program and State plan services, as well as needed medical, social, educational and other services. Our agency is approved under the New Jersey Department of Human Services Division of Developmental Disabilities.

Our goal is to ensure that all individuals are treated with care and respect. Using Community Based Support, we help assist the individual achieve his or her goals.

We proudly serve Bergen, Burlington, Camden, Essex, Hudson, Hunterdon, Mercer, Middlesex, Monmouth, Morris, Ocean, Passaic, Somerset, Sussex, Union and Warren Counties and will be expanding soon!

Our Services

Our support coordination services are designed to facilitate positive outcomes with our individuals by following a process that is centered around our individuals and their families. At Support Connection, we’re more than a switchboard connecting point A to point B. Using a Person Centered approach, we strive to understand and create a bond with individuals and families so we can match them up with the services that will best serve their needs.

We identify the individual's support needs.
We work with the individual and their family to develop the NJ Person Centered Planning Tool (PCPT) and NJ Individual Service Plan (NJISP).
We work with service providers to provision appropriate services for the individual based on funds available in the individual's budget.
We continuously monitor the individual's supports and services.
We are available to respond to emergencies and other service-related needs of the individual and/or family.

Our support coordinators strive to establish a bond with the individual and their family in order to truly understand each individual's unique situation. This allows us to coordinate the services that will best serve their specific needs.
